Referring a Patient to Balsam

We encourage you to introduce Balsam to your loved one prior to referring them to us. Involve them in decision-making, and let them value the care they will receive, so that they will welcome Balsam as a partner in their care.

How?
Inform them that the care that Balsam provides will offer them and their family an extra layer of support.. This care is committed to managing their symptoms, providing them with comfort, and improving their maximum potential functionality thus improving their overall quality of life. All this is done in collaboration with their primary physician (if the patient agrees to this).
You chose Balsam because you want to ease the burden of their illness and help them be comfortable at home. Balsam will also help you, as a family, learn how to better care for your loved one at home.

How to refer?
Please contact our office on 01-748574, Mondays through Fridays between 8:30am and 4pm. One of our team members will answer your call and ask you for some information regarding the patient. Within the next 48 hours (or, rarely, a little longer) you will receive a call from one of Balsam’s nurses to ask you for more information regarding the patient. This information will help the nurse know more about the patient, and be able to formulate a personalized plan

How do you know if Balsam is right for you?
If you feel you need more information to offer to the patient, or more information about Balsam’s services, please do not hesitate to call us. We will help you decide what is the most suitable path for you and your loved one after gathering necessary information
